Request for Return of Firearm

  • This form must be completed and submitted to the Madison County Sheriff's Office Crime Scene Investigations division. This procedure must be completed prior to the release of all firearms.
  • You will be contacted within 30 days of the receipt of this form by the Crime Scene Unit.
  • You must provide a photocopy or picture of a government issued photo-identification (driver's license, State ID card, Passport, etc.).
  • If the firearm(s) was seized from another person you must provide documented proof of ownership (receipt, bill of sale, etc.).
  • If the owner of the firearm is deceased, it can only be returned to the next of kin. A background check will be completed prior to the release of all firearms. A copy of the death certificate may be required.
  • If you have been convicted of a felony crime or domestic violence in any state, you will not be eligible to receive a firearm. If you have any pending charges regardless of misdemeanor or felony status your firearm may not be eligible for release.

 

 

Request for Return of Property  

(Anything other than a firearm)

  • Complete and submit this form to request the return of property.
  • You must provide a photocopy or picture of a government issued photo-identification (driver's license, State ID card, Passport, etc.).
  • Once this form is received the Crime Scene Unit will verify with the assigned criminal investigator that the property is cleared for release. You will be contacted within 30 days of the receipt of this form by the Crime Scene Unit.
  • If the property was seized from another person you must provide documented proof of ownership, or owner's permission to receive the property by completing the Request for Transfer of Property form also.

 

 

Request for Transfer of Property

  • If you are requesting property be released to another individual who is not the owner of the property, please complete this form.
    • An example would be someone who is prohibited from possessing a firearm may use this form to transfer the firearm to a dealer or a non-prohibited person.
  • You must provide a photocopy of a government issued photo-identification (driver's license, State ID card, Passport, etc.).
  • This form must be printed off and notarized.
  • Once completed it can be submitted in person or by mail to the address on the form.
  • The person receiving the property/ firearm must complete the appropriate request form above.
